Carolina Regional Orthopaedics will be glad to assist you
with your disability forms. You can either bring them to our office or have it
faxed but the most important thing to remember is that it must be signed by you
giving us permission to complete it. You must pay in advance to have your form
completed. We charge $10 per form unless you have had surgery. If you have had
surgery at our facility we will waive the $10 fee during your global period
(which could be anywhere from 15 to 90 days). The global days depend on what
type of surgery you are having or had. It will take 7 to 10 days to complete
your disability form. After your form is completed we will either fax or mail
it to your insurance company, mail it back to you or you can pick it up from
our office. You are welcome to check with our office at anytime to check the
status. We understand how important this is to you and we will do everything we
can to get this done quickly.
